Based on this, it really does sound like an adapter (card) in system may
be loose.

Might want to take a maintenance, power down system, open up, firmly
re-seat each adapter, close up, power on and boot.

This can happen if an adapter is not anchored to the system well. I have
had this happen in the past with a *mostly* inserted adapter but that
was slightly loose with several pins due to tugging on the attached
cables. Causing the PCI bus scan to return odd results, and the error.
