What machine? What fw level (could be checked with the comand lsmcode)

I faced the similar error on p5-520 machines with firmware ~ SFXXX_075
(version of 0503 fixed this - at least this error was not reported in
errorlog and p5 system seems to be functioning correctly for the last 9
months).

You can go to http://www.ibm.com and in enter SCAN_ERROR_CHRP in search
string and check the results.
