I've now managed to get alsa running with my sound card and joystick but
now I've lost the sound on my TV card!
It uses the bt87x sound module and I've added the option to make.conf

ALSA_CARDS="ens1371,bt87x"

I've edited /etc/modules.d/alsa

but it still doesn't work.

It was listed when I did emerge alsa-driver with out errors but still no
joy. The strange thing is I get a picture but no audio. There is also no
option in kmix for the bt87x.

Any ideas, because I'm fresh out of them!

I had issues with alsa-driver as well, so I just unmerged it and instead
compiled it directly into the kernel. Not really a solution, I know, but
this was the easiest way to go for me. It's worth a try.

I don't know about kmix, maybe you should try alsamixer.
Read the manpage to find out how to select the correct card.
Also, check if ALSA lists your cards at all.
