I've altered the instructions slightly on this:

1. Go to https://www.paypal.com

2. Click the 'Security Center' link at the bottom of the page

3. Click 'Report a Problem'

4. Select the proper Topic and Subtopic related to your issue

5. Complete appropriate webform and click 'Submit'
