Default issue in configuring the Sunstoregde T3 with sun fireV880

Hi Gurus

I have a problem in configuring Sun Storegde T3 with Sun fire v880
server.
we have qlc2200 Fc HBA to connect to T3
It behaves randomly i.e : sometimes when i do a probe in ok prompt it
shows the disks(vol in t3) .
after sometime it doesn't.
Likewise if i boot the solaris os and run format it takes more seconds
to show the t3 disks and also the status on disks as <formatting>..
then later this disks are also not seen in format utility.
The behaviour is random..
I applied the latest solaris pathes and SAN patches
I upgarde the t3 firmware also.
still i face this issues

can anyone help me to get-out of this circle..

Do you have some messages (online/offline) under /var/adm/messages ?
Cables OK? FC-Plugs are ok and clean?
