Default SunPCI crash

Hi,

I have an older version of SunPCI (1.2.2) running on an Ultra 5 with
Solaris 2.6. I've been running it for years, but now it crashes every
time. The SunPCI window comes up, Windows NT 4.0 tries to boot, and I
get:

WARNING: Your drive may be corrupt. Please let AUTOCHK run.
<warnings not to interrupt AUTOCHK>
CHKDSK is verifying files and directories.

CHKDSK then completes, with no errors being displayed. The screen
then turns beige, as though it's going to display the Windows desktop,
but then the SunPCI window vanishes.

Any ideas? Looks like my disk image may be corrupt.
